Can it be used in saltwater?

No, this portable shower is designed for use with fresh water only.
If you notice reduced water flow, it may be caused by blockages inside the showerhead or at the bottom of the pump. You can unscrew the showerhead to clean the inside, or open the round mesh cover at the pump bottom to remove debris.

Does it come with a carrying case?

A separate storage bag is not included. However, this is a complete shower set that already comes with:
